A plastic spoon in your brain is a very good sentence. It is vivid, it is frightening, and it is the reason most people have heard of this study. It is also not in the study.

The claim

“There is a plastic spoon's worth of microplastic in your brain, and it is causing dementia.”

Everywhere since February 2025, on podcasts, in wellness marketing, and in headlines. Usually delivered as two facts. It is one measurement and one thing the researchers explicitly declined to conclude.

The plastic is really there and the paper is real. The spoon is an interview, and the paper's own text rules out the causal claim built on top of it.

Real finding, wrong scope. Correct in part, misapplied in practice.

The spoon is not in the paper

This is the load-bearing fact and it takes one search of the text to check.

The published paper reports concentrations in micrograms per gram of tissue. It never multiplies that figure by the weight of a brain, and it never reaches a number of grams. The words plastic spoon do not appear anywhere in it.

The comparison came from the study's senior author in press interviews on the day of publication, and it is the version that travelled. It does not appear in the university's own press release either.

Scaling a per-gram laboratory measurement into a household object is a real step with real assumptions, and it is the step that made this study famous. It is worth knowing that the step happened in an interview rather than in the peer-reviewed text, because the peer review covered one and not the other.

What was actually measured

Strip the spoon and there is a specific, checkable set of numbers.

The paper reports a median of 3,345 micrograms per gram in brain samples from 2016 and 4,917 micrograms per gram in samples from 2024, with roughly 20 to 28 people at each timepoint. The comparable 2024 medians for liver and kidney were 433 and 404.

So the brain figures are several times higher than the other organs tested, and about 50% higher in 2024 than in 2016. Those are the findings. They are interesting, and they are not a spoon.

The paper also carries an Author Correction from March 2025 covering duplicated supplementary panels, two mis-sized scale bars and an omitted sample-preparation step. Corrections happen. We note it because we would want it noted about us.

The dementia number, and the sentence printed underneath it

The causal half of the claim is contradicted by the paper it comes from.

Twelve brains with a documented dementia diagnosis showed a median of 26,076 micrograms per gram, far above any of the normal cohorts. That is a large difference and it is why this study became a dementia story.

Immediately after reporting it, the authors write that atrophy, impaired blood-brain barrier integrity and poor clearance are hallmarks of dementia and would be expected to raise these concentrations, and state that “no causality is assumed from these findings.” The conclusions repeat that the data are associative.

Read that carefully, because it is unusual. The authors are not merely declining to claim causation. They are naming the reverse explanation and saying it is what they would anticipate: a brain damaged by dementia may clear particles less well, so it accumulates more. Same association, opposite arrow.

There is no matched control group for those twelve brains. They are compared against pooled cohorts collected differently. That is a limitation, not a scandal, but it is why twelve is doing a great deal of work here.

The measurement itself is contested, in the same journal

This is not a fringe objection. It went through the journal.

In November 2025, Nature Medicine published a formal comment from nine researchers across institutions in Italy, Finland, Germany, the Netherlands and the United Kingdom, arguing the work faces methodological challenges including “limited contamination controls and lack of validation steps.”

The technical concern that specialists keep returning to is that brain tissue is unusually fatty, and that fats can break down under this analysis into fragments resembling those from polyethylene, the polymer the study found most of. The paper anticipates this objection and argues its digestion method reduces the risk.

The authors published a reply the same day, framing the exchange as being about uncertainties in even the best current methods. That is a fair description and it is also an admission that the absolute number is not settled.

The internal cross-check often cited as replication was five samples re-run by collaborators who are co-authors on the paper, using the same technique. That tests whether the method is reproducible. It does not test whether the method is measuring what it thinks it is measuring.

What holds up, and it is not nothing

The verdict is Half True because half of it is true.

Microplastic in human brain tissue is not in doubt. In 2026, an unaffiliated group in China reported finding it in the overwhelming majority of samples they examined, in Nature Health. Presence is established.

The upward trend between 2016 and 2024 is a genuinely important finding if it survives, and the dementia association is worth pursuing hard, whichever direction the arrow turns out to point.

None of that requires a spoon, and none of it requires causation. The honest version is that we are accumulating plastic in our tissue, nobody yet knows what it does, and the one group that looked at dementia said plainly that they were not claiming it causes anything.

What would change our mind

Stated in advance so the score can be held to it.

An independent laboratory reproducing the mass concentration with controls for the fat interference problem would move Replication sharply down and take several points off this score.

A study designed to separate the two directions, for example measuring concentrations before dementia onset, would settle the part that matters. Until one exists, anyone telling you plastic causes dementia is going further than the people who did the work.

How it resolves. Resolves HELD if no such peer-reviewed figure from an unaffiliated group exists by January 31, 2027. Resolves MISSED if one is published, whatever it shows, since the point of the call is whether anyone else can measure this at all. Reports of presence without a mass concentration do not count either way. Resolves VOID if the original paper is retracted, which would make the comparison meaningless.

This is a judgment call, and it goes on the record either way.

  2. Treat presence and effect as two separate questions. The first is established. The second is open, and the researchers say so themselves.
